Adults

Adult social work and social care services are the responsibility of the Aberdeen City Health and Social Care Partnership.  Almost all adult social care in the city, with the exception of some in-house Learning Disability services are provided by the third and independent sectors.

Self-directed support covers all adult social work and social care services with the exception of our statutory justice social work service.

Following on from a referral, our social work practitioners will undertake an assessment of an individual’s needs and current circumstances; in doing so, they will explain our eligibility criteria and if needs be signpost to other more appropriate sources of advice and support. For those individuals who are eligible for a service they will provide appropriate advice and support to enable the individual to choose their preferred SDS option and agree the personal outcomes that will delivered by their selected SDS arrangements.
